2019-2021 Application Form

To apply for the Lavin Entrepreneurship Program, please complete this application in its entirety. You must be attending SDSU in Fall 2019 as a student and plan to graduate in May, 2021 or later. An incomplete application will not be considered. The final submission date for the Lavin Entrepreneur Program is June 21th, 2019.

Take the necessary time to read through the application before starting. Before submitting your application, ensure that you have carefully considered your responses, completed each section and answered each question thoroughly. Good Luck!
